Get your Supervisor's signature;

Step 3

Scan the Research NDA and OSHE Declaration Form and rename the form as e.g. NDA_;

Step 4

Fill in the online membership form [eFORM] with all the required info and attachments;

Step 5

Please meet Dr. Arif to activate your access card after form submission.
